Amidst an uptick in the number of pregnant people dying from COVID-19, the Centers for Disease Control and Prevention on Wednesday issued an urgent health advisory to doctors, public health leaders, and babymakers across the country - urging more vaccinations of pregnant people to protect the lives of those who are or may become pregnant, and to keep their fetuses safe and healthy too.

大腸癌是常見癌症，包括資深藝人賀一航、綜藝祖師爺豬哥亮、Ｍakiyo媽媽、劇場大師李國修、心海羅盤主持人「葉教授」等，還有漫威英雄片「黑豹」主角查德威克鮑斯曼，都因不敵大腸癌而辭世。衛福部去年底公布，大腸直腸癌每年新增1萬5千例案例，連續十三年居十大癌症之首。

根據「元氣周報」健保大數據系列 「大腸癌0到４期最新治療指南」，醫師提醒，大腸癌可以透過糞便潛血檢查早期篩檢，而肥胖與缺乏運動是大腸癌危險因子，國人飲食西化加上蔬菜水果攝取少，又久坐、缺乏運動、晚睡等，腸道蠕動慢、排便不順，更增加致癌物與腸道接觸機會。

「元氣周報」健保大數據系列：「大腸癌0到4期最新治療指南」，報導如下：台大醫院內科部主治醫師暨健康管理中心主任邱瀚模表示，大腸癌早期無明顯症狀，若出現大便帶血或體重減輕等，多數已非早期。不過，大腸癌與年齡、人口老化、生活環境、飲食西化、家族史、缺少運動、吸菸等因素有關，可藉由定期篩檢早期發現。

邱瀚模說，台灣每年新發生1萬5千名大腸癌患者中，透過篩檢發現占三分之一，呼籲50歲至75歲民眾把握機會，每兩年接受定期篩檢。定期篩檢率若年提升到五成甚至七成，大腸癌死亡率可望降到更低。若檢出疑似大腸癌，至醫院做進一步確診、治療並定期追蹤，存活率相當高。

邱瀚模說，大腸癌成因很多，男女發生率差不多，雖可能發生在任何年齡，但好發50至85歲，年齡愈大，瘜肉愈容易在大腸各處生長，增加癌變風險。若以完全沒做過篩檢的年長者來說，每15人就有1人可能有大腸癌。

民眾若有家族史，如一、二親等中的父母、兄弟姊妹中有人罹患大腸癌，罹癌機率會增加；長期吸菸者，罹患大腸癌機率也比非吸菸者高。若有糖尿病、高血脂、高血壓等，也增加大腸癌風險。

邱瀚模說，肥胖與缺乏運動是大腸癌危險因子。過去研究發現，肥胖、慢性發炎與大腸癌形成有密切關係。國人飲食西化又愛吃夜市食物，外食、速食吃進過多紅肉，夜市中熱狗、雞排多為高脂及高溫油炸物，加上蔬菜水果攝取少，又久坐、缺乏運動、晚睡等，腸道蠕動慢、排便不順，增加致癌物與腸道接觸機會。

台安醫院大腸直腸外科主任糠榮誠表示，研究顯示，紅肉如豬、牛、羊攝取過多，容易罹患大腸癌，建議紅肉一天攝取量不宜超過16克，若吃牛排餐，則一周最好不要超過兩次。

那麼吃素是否可預防大腸癌？糠榮誠表示，多吃蔬菜有助腸道健康，降低罹癌的可能性，但不代表「吃素」可以預防大腸癌。他表示，素食多使用植物油，這類油品不耐高溫，反而可能因烹調方式有致癌風險。另外，許多素食是豆製加工品，建議以原形食物為宜。

糠榮誠和邱瀚模皆表示，預防大腸癌，應保持規律作息、飲食均衡，少吃油炸物、紅肉或煙燻、鹽漬加工肉品，並養成定期運動習慣，注意體重控制。最重要的便是定期接受大腸癌篩檢，目前成人健檢提供50至74歲民眾兩年一次免費糞便潛血檢查。

1.糞便潛血檢查   時序已入冬，研究顯示，冬天是做糞便潛血檢查最合適的季節。邱瀚模表示，韓國今年七月分析三萬五千多名民眾的糞便潛血檢查及大腸鏡檢查結果，發現當環境溫度高於24度、濕度高於80%，糞便潛血檢查陽性率及敏感度顯著下降，這表示發現癌前病變機會降低，導致民眾下次篩檢前發生大腸癌機率上升。

邱瀚模說明，並不是夏天不能做糞便潛血檢查，只是高溫高濕讓檢體容易變質，破壞血紅素，降低陽性率。建議民眾一定要把檢體盒放冰箱，冷藏可以保存一周都沒問題。但很多民眾很難接受冰箱有糞便檢體，都放在室溫環境，如果夏天放一個晚上就立刻送驗，可能影響不大，如果幾天後才送驗，檢體品質就會打折扣。

2.大腸鏡檢查  現行糞便潛血檢查相當精準，若檢查結果是陽性，一定要接受大腸鏡檢查。大腸鏡檢查是將大腸鏡管子從肛門進入直腸，經乙狀結腸、降結腸、橫結腸、升結腸直至盲腸。檢查過程中透過影像傳遞至電腦，醫師可即時診斷大腸內壁有否異常或病變。

邱瀚模表示，陽性個案未作大腸鏡，未來死於大腸癌風險增加64%。不過，大腸鏡的執行效果，受到盲腸到達率、腺瘤偵測率、清腸程度影響。

邱瀚模建議，如果大腸鏡沒有完整檢查到盲腸，下次篩檢前發生大腸癌風險增加76%，所以建議偵測大腸癌一定要做完整全程的大腸鏡，不宜只做直腸鏡或乙狀結腸鏡。如果在腺瘤偵測率小於15%的院所做大腸鏡，下次篩檢前發生癌症的風險增為2.4倍。

至於清腸程度，建議確實執行低渣飲食，並在檢查前五到八小時喝清腸藥，太晚或太早喝都會影響清腸率。

When floods hit southern India, it looked as though a young couple would be unable to make their wedding. But they managed to reach the temple by floating through the flooded streets in a giant metal cooking pot.

當洪水侵襲印度南部，一對年輕小倆口看似無法辦婚禮了。但是他們置身於一個巨大的金屬烹飪鍋中，通過被淹沒的街道，設法抵達寺廟。

The couple, identified by local media as health workers in the town of Alappuzha in southern India’s Kerala state, could be seen smiling and waving in their ceremonial clothes, as two men waded through hip-high waters to steer their pot to the temple.
